It acts on specific changes of ageing around the eyes: loss of elasticity, fine lines, bags from orbital fat, weakening of the orbicularis and drooping of the upper eyelid.
It does not change the shape of the eye or the character of the face: the effect reads as natural rejuvenation, not as a radical change.
It corrects a facial expression that does not match how you feel — sadness, tiredness, annoyance — and improves the proportion between the thirds of the face.
The result lasts for years, but surgery does not stop ageing: a touch-up may be needed if new laxity or a build-up of fat appears.
It is often combined with botulinum toxin, hyaluronic acid or fat transfer into the tear trough, and fractional laser or a peel, to treat what surgery alone does not reach.
Blepharoplasty is an operation within plastic surgery that removes excess skin, fat and, in some cases, muscle from the upper and lower eyelids. Its main purpose is to rejuvenate the eyes and bring the eye region into harmony with the rest of the face. Although the operation can also have functional indications where the excess skin affects vision, in most cases the motivation is aesthetic.
At the Instituto de Cirugía Plástica del Mediterráneo, in Valencia, this operation is carried out to the strictest standards of medical quality. An individual approach is applied to achieve natural results adapted to each patient’s anatomy, with one clear aim: to improve the facial expression without altering the identity of the face.
The benefits are better understood once you know what the surgery corrects and who it is indicated for, in the general guide to blepharoplasty.

Over time, the eyelids undergo a series of changes related to the ageing of the skin and the loss of structural support:
The skin loses its elasticity and firmness.
Folds and fine lines appear.
Bags form from a build-up of orbital fat.
The orbicularis muscle can weaken.
The upper eyelid can drop, giving a tired appearance.
In the lower eyelid, laxity can produce a sad or weary look.
These signs make the facial expression look aged, even in younger people. Blepharoplasty acts directly on these changes, giving the face back a rested, bright and harmonious appearance.
Visible rejuvenation of the eyes
The main aesthetic benefit of blepharoplasty is the rejuvenation of the eyes. This area is one of the first places where the signs of age show. Operating on the eyelids achieves a notable change with no need to alter other parts of the face. Among the effects patients value most:
Removal of the bags under the eyes.
Improved tightness of the skin of the eyelids.
Fewer lines around the eyes.
Restoration of the natural crease of the upper eyelid.
Removal of that chronically tired look.
Greater facial symmetry.
An enhanced contour around the eye with no loss of a natural appearance.
It is important to note that blepharoplasty does not change the shape of the eye or alter the character of the face. Its effect reads as natural rejuvenation, not as a radical change. That is why it is one of the procedures most requested by patients who want discreet but effective results.
Improvements in facial proportion
The eyes are a focal point in how a face is perceived. Any disproportion in the eyelids affects the overall harmony. Removing the surplus skin and the bags improves the relationship between the thirds of the face, which favours a more attractive visual balance.
What is more, by restoring the opening of the eye and clearing the area around it, the result is:
A clearer, more expressive face.
Brighter, more present eyes.
Less of the shadow the bags create, which improves facial tone.
Changes of this kind have a direct effect on how others perceive the person, which often also helps improve the patient’s self-esteem.
What the patient notices and what other people notice
The aesthetic benefit of a blepharoplasty can be measured in two ways, and it is worth looking at both, because they do not always agree. With this surgery, they do.
From the inside. A prospective multicentre study followed 2,134 upper eyelid patients with the FACE-Q questionnaire, the validated tool for this kind of surgery. At six months satisfaction with the eyes had risen by 48 points out of 100, psychological wellbeing by 11 and appraisal of one’s own age by 22: on average, patients saw themselves as 3.3 years younger. At one year the improvement was still there.
From the outside. What someone feels about their own face and what the person looking at it perceives are different things, so some studies put the question to independent observers. In one of them, 203 people unconnected with the study rated before-and-after photographs without knowing what for: the age they attributed to the face fell from 51.8 to 42.1 years, and the ratings for attractiveness, health and perceived success rose too.
It is a short series — twelve patients — and that should be said, but it points in the same direction as the two thousand in the earlier study: the change does not stay with how the patient feels, it shows from outside. And what shows is not the surgery; it is the rest.
Natural, lasting results
At the Instituto de Cirugía Plástica del Mediterráneo, every operation is planned taking into account not only the current state of the eyelids but also the genetic characteristics, the facial structure and the patient’s realistic expectations. That makes natural results possible, respecting each individual’s features.
The effects of a blepharoplasty can last for many years, particularly if the patient adopts good habits and looks after their skin. Although surgery does not stop ageing, it does remove its most visible signs in the eye region, and these do not usually reappear with the same intensity.
Aesthetic benefits in men and women
Although aesthetic concerns tend to be associated more with female patients, more and more men are choosing blepharoplasty. In both cases the operation has to be adapted to the facial characteristics proper to each sex in order to keep a natural look.
In women the aim is more open eyes, without over-tightening or altering the natural crease of the eyelid.
In men a straighter, firmer structure is preserved, avoiding changes that would feminise the contour of the eye.
The surgery achieves a rejuvenating effect with no obvious signs that an operation has taken place.
Correcting a negative facial expression
One of the most frequent reasons patients come to the clinic is that they feel their face reflects emotions that do not match how they feel. A build-up of fat or laxity in the eyelids can produce an appearance of:
Sadness.
Tiredness.
Annoyance.
A lack of energy.
Correcting these changes brings the facial expression back into line with the patient’s personality. That improves not only their outward appearance but also the way they relate to other people.
Combining it with other aesthetic treatments
Blepharoplasty can be carried out as a single procedure or as part of an overall facial rejuvenation. It is frequently combined with:
Botulinum toxin, to soften crow’s feet and lift the tail of the brow.
Fat transfer (fat grafting) or hyaluronic acid in the tear trough, to improve the transition between eyelid and cheek.
Fractional laser or a peel, to treat fine lines and improve the texture of the skin.
Combining them gives a more complete result, acting on all the elements that affect the appearance of the upper third of the face.
Not all blepharoplasties are the same. There are many variables to be considered in defining the most suitable surgical plan. Among them:
Age and skin type.
The degree of skin laxity.
The amount and distribution of orbital fat.
Muscle tone and the state of the outer corner.
Any history of previous surgery or medical conditions.
On the basis of these, the surgeon will recommend an upper, lower or combined blepharoplasty, and whether the external or the transconjunctival technique is used.
Choosing a specialist centre
The aesthetic benefits of blepharoplasty are only achieved if the operation is carried out by a plastic surgeon with specific training in facial surgery. At the Instituto de Cirugía Plástica del Mediterráneo, procedures are carried out by accredited professionals with demonstrable experience and a medical approach based on safety, technical rigour and quality of care.
The centre has fully equipped facilities, rigorous postoperative follow-up and an approach centred on the patient. Each case is analysed individually to ensure the results meet the highest medical and aesthetic standards.
Blepharoplasty is not a superficial procedure or a passing fashion. It is surgery with anatomical and functional foundations that, well indicated and well carried out, can offer notable, lasting and deeply satisfying aesthetic benefits. If you feel your eyes reflect more age or tiredness than you feel, a medical assessment can help determine whether this operation is right for you.

What exactly does aesthetic blepharoplasty act on?
On the specific changes of periorbital ageing: the loss of skin elasticity and firmness, fine lines, bags from a build-up of orbital fat, weakening of the orbicularis muscle and the descent of the upper eyelid.
Will people be able to tell I have had surgery?
The aim is that they cannot. It does not change the shape of the eye or alter the character of the face: the effect is perceived as natural rejuvenation, not a radical change. It is one of the most requested procedures precisely among those who want discreet but effective results.
How much does patient satisfaction improve?
It has been measured. A multicentre prospective study followed 2,134 upper-eyelid patients with the FACE-Q questionnaire: at six months satisfaction with the eyes had risen by 48 points out of 100, psychological wellbeing by 11 and the appraisal of their own age by 22 — on average they saw themselves as 3.3 years younger. At one year the improvement was still there.
Do other people notice, or only the patient?
From outside too. In one study, 203 people unconnected with it rated before-and-after photographs without knowing why: the age they attributed to the face fell from 51.8 to 42.1 years, and the ratings of attractiveness, health and perceived success also rose. It is a short series — twelve patients — but it points the same way. What shows is not the surgery; it is the rest.
People say my face looks angry or sad when I don’t feel it. Does this correct that?
It is one of the most common reasons for consulting. A build-up of fat or slackness in the eyelids can give an appearance of sadness, tiredness, anger or lack of energy. Correcting those changes makes the facial expression match the person’s character again.
Does it improve the proportions of the face?
Yes. The eyes are a focal point in how a face is perceived, and removing the surplus skin and the bags improves the relationship between the facial thirds. The face looks clearer and more expressive, the gaze brighter, and the shadow cast by the bags is reduced, which improves the tone of the face.
Is the operation the same for a man and a woman?
No. In women a more open look is sought, without overdoing the tightening or altering the natural crease. In men a straighter, firmer structure is preserved, avoiding changes that feminise the eye contour.
How long do the results last?
They can last many years, especially with good lifestyle habits and skin care. Surgery does not stop ageing, but it removes its most visible signs in the eye region and these do not usually return with the same intensity. A touch-up may be needed if new slackness or a build-up of fat appears.
What other treatments is it combined with?
Often with botulinum toxin to soften crow’s feet and lift the tail of the brow, with fat transfer or hyaluronic acid in the under-eye area to improve the transition between eyelid and cheek, and with fractional laser or peeling for fine lines and skin texture. That covers what surgery alone does not reach.
Are all blepharoplasties the same?
No. The plan depends on age and skin type, the degree of skin laxity, the amount and distribution of orbital fat, the muscle tone and the state of the lateral canthus, and the history of previous surgery or medical conditions. From that follows whether an upper, lower or combined procedure is right, and whether the technique is external or transconjunctival.
