The profile: significant skin laxity on the inner thigh that does not improve with exercise, excess skin after major weight loss, localised fat that makes walking or dressing uncomfortable, and skin whose elasticity no longer allows it to retract on its own.
Medical criteria: stable general health, not being actively obese —ideally, a target weight held for at least 6 months—, not smoking or stopping four weeks before and after, and no clotting disorders or uncontrolled circulatory problems.
There is no strict age limit: what counts is the quality of the skin, the capacity to heal and understanding what the procedure can and cannot do.
Surgery improves the contour, but it does not remove cellulite and it is no substitute for exercise.
It is advised against with ongoing obesity or further weight loss planned, untreated body image disorders, decompensated chronic illness, pregnancy, breastfeeding or plans to conceive soon, and a tendency to keloid or hypertrophic scarring.
A thigh lift, also known as thighplasty, is an operation indicated to treat excess skin and fat on the inner thigh. Although it gives visible, lasting results, it is not recommended for everyone. Before considering the procedure, it is essential to look at whether the patient meets a set of medical and aesthetic criteria that make the surgery both feasible and safe.
At the Instituto de Cirugía Plástica del Mediterráneo, in Valencia, we carry out a thorough assessment to decide whether each person is a suitable candidate for a thigh lift, taking into account their anatomy, their state of health and their expectations.
Significant skin laxity on the inner thighs that does not improve with exercise or non-invasive treatments.
Excess skin after major weight loss, as happens in post-bariatric patients.
A localised build-up of fat that distorts the shape of the thigh and makes walking or dressing uncomfortable.
Disproportion between the lower body and the rest of the trunk, unbalancing the body’s outline.
Skin with reduced elasticity that cannot retract naturally.
In these cases, surgery removes the surplus tissue and redefines the inner thigh contour, with clear functional and aesthetic results.
Medical criteria for being a candidate
Beyond having an aesthetic indication, the patient must meet certain minimum medical conditions to make recovery safe and uncomplicated. The main criteria are:
Stable general health: the patient must be free of decompensated illness, active infection or serious systemic disorders.
A suitable body mass index (BMI): no exact weight is required, but it is essential that the patient is not actively obese. Ideally they will have reached their target weight and held it steadily for at least 6 months.
Not smoking, or committing to stopping at least four weeks before and after the operation, since tobacco impairs healing and raises the risk of complications.
No clotting disorders or uncontrolled circulatory problems, such as deep vein thrombosis or severe venous insufficiency.
Being able to follow medical advice after the operation, including wearing compression garments, relative rest and regular follow-up.
In patients with a significant surgical history or pre-existing illness, a further assessment is carried out together with the anaesthetic team.
Age and emotional stability
A thigh lift can be performed in younger people and in older patients alike, as long as their health allows it. There is no strict age limit, but it is important to consider:
The quality of the skin, which shapes the final result.
The capacity to heal, which can vary with age and with certain associated conditions.
The degree of emotional maturity needed to understand the scope, the limits and the risks of the procedure.
A key point in being considered a candidate is that the patient has realistic expectations and understands that surgery improves the body contour, but does not remove cellulite and is no substitute for physical exercise.
What makes a good candidate, according to the data
Of everything that can be assessed in a consultation, there are two factors the literature singles out by name because they are significantly associated with how the recovery goes. And the good news is that one of the two can be changed.
A 16-year experience covering 151 thigh lifts found that age was related to wound complications and that smoking was significantly associated with scar problems. It is the actionable factor in this operation: stopping smoking in the weeks before and after is not a leaflet recommendation, it is the most direct lever the patient has over their own result.
The same work clarifies the part weight plays, and it is not the one usually assumed. Body mass index does not rule anyone out: it guides the technique. Thighs with more volume to begin with —a mean BMI of 29.3 in that series— needed the scar extended down the inner surface, whereas at a mean of 25.1 the horizontal scar hidden in the groin was enough. That is why the question in the consultation is not “do I weigh little enough?”, but “what scar does this thigh need?”.
And on stable weight, which is the other classic requirement, there is a technical argument as well as common sense: adding liposuction to a thigh lift cuts complications from 59% to 13%, and that manoeuvre pays off when there is still fat to aspirate and skin is what is genuinely in excess. A thigh whose volume is still going to change is not at that point yet.
When it is not recommended
There are cases in which a thigh lift is not indicated, or must be postponed until the patient meets the necessary conditions. These situations include:
Ongoing obesity or further weight loss planned, since the skin may droop again.
People with untreated body image disorders (such as body dysmorphia), where surgery will not resolve the psychological conflict.
Decompensated chronic illness, such as poorly controlled diabetes, severe hypertension or cardiopulmonary disease without medical follow-up.
Women who are pregnant or breastfeeding, or who plan to conceive in the near future, because of the hormonal impact on the skin and the fatty tissue.
Patients with a tendency to abnormal scarring, such as keloids or hypertrophic scars.
In these cases, the advice is to optimise the medical conditions first, seek psychological support if needed, or choose other treatment options.
At the first consultation with a specialist plastic surgeon, the patient’s case is examined in detail through:
A review of the medical and surgical history.
An assessment of the type of laxity, the quality of the skin and the presence of localised fat.
A full physical examination, including the symmetry of both thighs, the elasticity and the thickness of the tissue.
Examination of the groin and the gluteal fold, where the incision sits.
The possible results are also explained, along with the characteristics of the scar, how long recovery takes and the aftercare required.
Why a tailored approach matters
Every patient has a unique anatomy and a different goal. That is why the indication for a thigh lift should not be generalised. In some cases a full thigh lift may be advised, while in others a partial technique is enough, also called a mini thigh lift, with a smaller scar.
It may also need to be combined with other procedures such as liposuction, depending on how the tissue is distributed and what the patient expects. This overall approach improves body proportion and enhances the aesthetic result as a whole.
Being a candidate for a thigh lift does not depend only on wanting to improve the contour of the thighs. It calls for meeting specific medical criteria, having a genuine aesthetic indication and being prepared to take on the surgery and the recovery with commitment.
At the Instituto de Cirugía Plástica del Mediterráneo we carry out a full medical assessment to establish whether this procedure is right for you. Our team works with rigour, medical ethics and surgical experience to give results that are harmonious, safe and in keeping with your anatomy.
